Update OS support in README / Makefile.install

Kick Debian Lenny, Ubuntu Hardy, and RHEL 5 / CentOS 5 to the curb.

Also change the name of the README to the more generic & instructive
"Installing OpenSRF" rather than "README for OpenSRF #.#.#".
