Customize login form for Laurentian LDAP login

Now that we're synced up with LDAP authentication, provide the
appropriate guidance on the login form.

Set ou_email_host via a hidden form variable.

Provide more rational inline help for OSUL login form.

Link to the OSUL password reset form on login failure
