LP#1390138: Clarify 2.6-2.7 upgrade path

The file for upgrading from 2.6 to 2.7 series changed to be specifically
2.6.3-2.7.0 (not just 2.6 to 2.7.0 but 2.6.3 to 2.7.0).

Additionally, alter note on reingesting to include that it might be required
post upgrade script running.
