LP1991590: Better order for survey admin fields

To test:
1. Press Admin > Local Admin
2. Press Surveys
3. Create a new survey.
4. Notice that the Start Date field comes before the End Date field.
5. Repeat steps 1-2.
6. Select a survey and edit selected.
7. Notice that the Start Date again comes before the End Date.
