Staff Client Customization: Patron Registration Form Tweaks

- Fixed alignment of checkboxes.
- Hid Claims returned on new patron registration form.
- Hid Claims never checked out.
- Made survey drop 'up' instead of dropdown so it
  doesn't disappear off of page.
