Get eresources working again in search results
The house of cards that had crashed to earth with the async
problems fixed in the previous commit also broke electronic
resources display in the search results for the LU and Algoma
skins. This sorts it out by simply moving the electronic
resources into their own row, rather than overloading the
local callnumber row.

Fix misalignment of SFX results to bib entries
Querying at the record level ensures that the async requests
don't mess up our attempt to synchronize the SFX results with
the bibs to which they belong.

Prevent renewal of an item which has a hold placed on it
Per http://biblio.laurentian.ca/tickets/conifer/wiki/devHolds, we had
this functional in 1.6 but must not have had the updated script checked
into SVN (now git) for the purposes of our upgrade, as the functionality
is not present after our upgrade.
Adding this to the repo so that we can test and hopefully get rid of the
regression.

Add a cache-munging script-generating script
Quickly pulled together a script to munge the CSS and JavaScript
for a given skin so that upgrades don't result in painful mismatches.
Warning: this Perl script generates a shell script which you must
then execute. It's not elegant in any way, it's based on the shell
commands that I issued by hand in a hurry. However, it does give
you the opportunity to examine what the script is going to do before
it munges your skin(s).

Adopt the more sophisticated subject handling from trunk
The core skin limits display of subject subfields to the a-z range,
which is useful as we begin to add subfield 0 via the authority

Only invoke BibTemplate if we have a reasonable number of copies
In the move from 1.6 -> 2.0, I missed this important tweak to conditionally use
BT on results page. Result was that slots were being rendered twice with
disastrous results. Now the result is much happier.

Get this_title_id inside the pertinent slot
BibTemplate (perhaps in combination with newer JavaScript engines) appears to
be rendering slots from the bottom up now, so curr_title_id was out of sync.
Grabbing the 901c from within the slot being rendered ensures that you've got
the right record ID.

IE really kicks it up a notch here; ugly.
It's still not clear why we seem to need to use raw DOM

A working rdetail_summary.xml for LUL skin
subjectMe() gets objects with .textContent properties that
BT.textContent() / dojox.data.dom.textContent() doesn't parse;
works fine in the inline implementation in the default skin
though.
Merge in the other 2.0-isms as we go.

Convert uniformly to Unicode output
LoC added content appears to come in ISO-8859-1, so decode that accordingly.
Then generate UTF8-encoded Unicode MARC records on output.

Add awareness of 856 $z as first subfield in Springer records
When $z is the first subfield in a Springer record, it generally means
that it's a public note of importance - for example, there may be two
856 fields, and the $z identifies which volume can be reached by each
URL.

Teach the ebook processing script how to turn LoC URLs into indexable content
There is a risk that we will duplicate content already in the MARC record,
but so far that doesn't appear to be the case. Also, we skip the
OCRed machine-generated tables of contents because their content is
entirely untrustworthy.

Do away with the 856 $z subfield as it currently simply duplicates link text
If we end up with a situation where we want a public note, then we can
easily reconstitute this. Until then, less code == mo better.

Just use the literal value of the supplied publisher
Example that I was given had "(Firm)" at the end of the publisher
name, but apparently that was a one-off and not intended to be
applied to all publishers. Okay.

Needs some massive refactoring for a smarter, more object-oriented approach.
But some days you just need to bash out something that mostly works.
