Be more defensive when parsing SFX target results

SFX in the wild has been observed returning targets for getFullTxt
requests that have no target URLs, which makes it rather impossible
to provide a link to the full text resource. Skip such targets (and
for good measure, skip them if they don't have a name either).

Also, change from the SID "conifer" to the more generic and accurate
"evergreen".

re-enable volume creation without items (and volume editing).  Previously, if multiple volumes with the same label were specified for a single given org, they would essentially get merged into a single volume.  This still happens, but with things like class, prefix, and suffix, the behavior is undefined if these are set on multiple rows for a "single" volume.  FIXME

modify the class and affix menus for an existing call number the _first time_ it is loaded in the interface.  some other minor code cleanup

wrong event, some debug aids, and toward setting class, prefix and suffix menus when retrieving an existing call number

change result sent back from open-ils.cat.call_number.find_or_create to include whether the call number was created or already existed.  this may be useful down the line for cleaning up vivicated call numbers

Fix cloning of AutoField widgets: LP #728055

Cloning Action/Trigger event definitions was not working properly, as
all values were being copied from the original object using their display
strings rather than the underlying values.

Bill Erickson suggested a fix in IRC that works, so full credit to him
