Eliminate duplicate call to get list of Funds

Moved to a dojo tooltip + dropdown-button menu for the top-level navigation

moved the picklist loading logic into a function which is then called via dojo.addOnLoad.  doing this so dojo will have time to parse the page and create the grid object before the picklist is retrieved

the previous asset creation code naively ignored the lineitem_detail phase.  new code takes volume/copy info from the lineitem_detail instead of directly from the client.  code still needs testing

Initial code to take a lineitem and create the necessary bib, call_number, and copy objects.
Currently, it's relatively free-form and allows for a lot of options from the client end
Probably need to add some level of user/org settings checking for various bits
