Add list of initial Board members; use "Board" instead of "organization"

"Organization" was a little confusing in the context of a member
project of an umbrella 501(c)(3).

Add clarification that terms of Fiscal Sponsorship Agreement prevail

Per Bradley Kuhn, we need to clarify that the scope of the Rules of Governance
is limited to the Evergreen Oversight Board, and do not extend to the Fiscal
Sponsor (Software Freedom Conservancy).

Add a table of contents and section numbering to the HTML output

For a formal document, you generally want a table of contents with numbered
sections to make referencing sections easier.
